Key Responsibilities\n

As an LCSW at Heartstrings & Wellness Mental Health, your day-to-day responsibilities will include:

• Providing both individual and group counseling aimed at assisting patients with coping strategies related to adjustment, grief, and other emotional challenges.

• Collaborating closely with medical teams, including physicians and nurses, to develop integrated treatment plans that address each patient's unique circumstances.

• Coordinating with community organizations to arrange post-discharge services, ensuring seamless transitions and continuity of care.

• Offering emotional support and education to patients and their families regarding comprehensive treatment options, community resources, and navigating the healthcare system.

• Providing crisis intervention and immediate counseling support when patients disclose urgent psychosocial issues.

• Documenting patient interactions, assessments, and treatment plans accurately and in compliance with organizational policies and legal standards.

• Participating in organizational committees, bringing expertise on social work issues, and contributing to quality improvement initiatives.

Patient Population / Client Focus\n

You will primarily serve individuals from varied backgrounds in the Portland, Oregon area who are accessing our community health clinic.
