Key Responsibilities\n

• Conduct comprehensive evaluations and assessments of patients to determine their needs and eligibility for hospice services.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSPs), ensuring personalized strategies that address the specific circumstances of patients and their families.

• Monitor and evaluate outcomes as a crucial team member, maintaining high standards of care and compliance with federal and state regulations.

• Develop rapport with families, providing essential emotional support and information about available resources to assist them in their caregiving journey.

• Utilize evidence-based practices and Natural Learning Environment practices to enhance the care provided to both patients and families.

• Serve as a liaison to connect families with available community resources, ensuring comprehensive care and support.
\n
• Employ the coaching model and natural environment teaching methods to facilitate skill development of both patients and family members.

• Collaborate with other healthcare professionals to share insights and best practices that contribute to improving patient outcomes.

Patient Population / Client Focus\n

This role primarily focuses on families dealing with seriously ill patients within a hospice setting. You will provide psychosocial support for both the patients and their loved ones, helping them navigate the emotional challenges associated with end-of-life care. Qualifications\n

• Master’s Degree or Doctorate in Social Work from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW) in Florida.

Compensation and Benefits\n
