Key Responsibilities\n

• Provide regular clinical supervision and support to practitioners delivering behavioral health interventions, ensuring adherence to clinical standards and policies.

• Conduct comprehensive clinical assessments, identifying patients’ psychosocial needs and formulating appropriate intervention plans tailored to individual circumstances.
\n
• Respond effectively to crisis situations, including child abuse cases and mental health emergencies, ensuring immediate support and intervention when necessary.
\n
• Research, refer, and advocate for appropriate community resources such as healthcare services, food assistance, and childcare to enhance clients' well-being.
\n
• Monitor clients’ progress post-intervention, assessing changes in their situations and following up to ensure ongoing support and improvement.
\n
• Facilitate group and family therapy sessions, providing education and coping strategies to support families dealing with grief, loss, and ongoing health issues.
\n
• Collaborate with interdisciplinary teams to ensure holistic care for patients, sharing insights and recommendations based on individual assessments.

Patient Population / Client Focus\n

In this role, you will primarily work with patients facing terminal illnesses, as well as their families. Your expertise will be instrumental in providing emotional support and practical assistance to those coping with grief, anxiety, and other related challenges. Your ability to understand the unique needs of this population will be crucial in fostering a compassionate and supportive environment.

Qualifications\n

• A Master’s Degree in Social Work from an accredited institution is required.

• A valid California Licensed Clinical Social Worker (LCSW) license is mandatory for this position.

• At least two years of experience in a behavioral health setting, with a focus on hospice or palliative care preferred.

• Demonstrated experience working with veterans and understanding their unique psychosocial needs.

• Proven ability to conduct psychosocial histories and perform case management evaluations, including effective interview techniques.

Compensation and Benefits\n
