Key Responsibilities\n

As a pivotal member of our counseling team, your responsibilities will include:

• Conducting continuous evaluations of patients to tailor treatment plans according to their unique needs and progress.

• Facilitating open lines of communication between supervisors, direct care staff, family members, and other pertinent stakeholders to monitor patient development.

• Drafting comprehensive schedules that outline appointments, home visits, and engagement sessions to keep track of service delivery and progress.

• Coordinating home visits for thorough observations, chart audits, and interviews with caregivers to gather necessary data for treatment plans.

• Implementing program strategies effectively, ensuring compliance with established protocols while updating documentation as needed.

• Engaging in interdisciplinary meetings and discussions to share insights and collaborate on patient care objectives.

Compensation and Benefits\n
