Work Environment

This physical therapist role is primarily onsite and involves providing care directly in patients' homes within the West Palm Beach area. Key Responsibilities

• Conduct thorough evaluations by reviewing patient histories, performing detailed assessments, and documenting results clearly for ongoing treatment plans.

• Communicate evaluation findings and proposed treatment plans effectively to patients and their families, incorporating their feedback into goal setting.

• Implement individualized treatment protocols based on patient evaluations and continually assess patient progress, modifying treatment plans as required.

• Collaborate with other healthcare professionals to optimize patient care, and recognize significant changes in patient conditions that require adjustments in therapy.

• Handle discharge planning efficiently, ensuring all patient care activities are appropriately documented and charged, maintaining compliance with regulatory standards.

• Participate actively in team meetings, adhere to productivity expectations, and help remove barriers to accessing care for patients.

• Engage in community outreach initiatives and marketing activities to promote the clinic's services and foster patient trust.

• Take the initiative to pursue continuing education and professional development, mentoring students or less experienced staff as applicable.

Patient Population / Client Focus

As a physical therapist in our home care setting, you will be working with a diverse patient population, including individuals recovering from surgery, those with chronic pain conditions, sports injuries, and elderly patients requiring rehabilitation. Your approach to care should emphasize compassion, empathy, and personalized treatment, ensuring that each patient feels valued and understood.

Qualifications

• Bachelor's degree in Physical Therapy from an accredited institution.

• Active state license to practice as a Physical Therapist, as required by the regulations in Florida.

• Current certification in Basic Life Support (BLS) from the American Heart Association is mandatory.

• Prior work experience is not necessary; we welcome new graduates who are eager to learn and contribute!

Preferred Experience

Compensation and Benefits
