Responsibilities Lead and collaborate with the care team to manage and deliver patient care plans.
Provide home‑health registered nurse case management for patients.
Document and chart patient care within critical deadlines.
Self‑manage time and schedule, including up to 20% travel to patient locations.
Qualifications Valid RN license in the state of employment.
Graduated from an approved school of practical nursing.
At least one year of RN experience and clinical competence in patient care.
Current driver’s license and ability to travel approximately 20% of the week to patient sites.
Proficiency with tablets, mobile phones, and EMR software.
