Key Responsibilities\n

• Advocate for patient rights and ethical decision-making, providing necessary support to patients and their families in times of difficulty.

• Document and ensure appropriate reporting of any vulnerability or suspected abuse as mandated by law, upholding the highest standards of safety and ethics.

• Design and participate in transition conferences with patients and families to ensure a smooth continuum of care.
\n
• Employ technology to identify and monitor patients at risk for readmission, utilizing evidence-based tools to facilitate timely interventions.

• Provide comprehensive support and counseling services, addressing both emotional and practical challenges faced by patients and their caregivers.

• Utilize electronic medical records to track patient progress effectively, maintaining clear communication regarding treatment goals and social work plans.

• Collaboration with an interdisciplinary team to deliver integrated care plans and address any anticipated needs of patients promptly.

• Coordinate care with community resources, ensuring patients have access to essential services based on their unique circumstances.

• Facilitate an empowering environment for patients and families regarding transition plans, ensuring they are fully informed and supported throughout the process.

• Provide information and assistance for any identified social or financial needs that affect patient health outcomes.

• Ensemble other duties as assigned to further support the team and enhance patient care.

Patient Population / Client Focus\n

Your primary focus will be on individuals grappling with a spectrum of mental health issues as a result of their medical conditions. This may include working with those facing severe illnesses, their families, and caregivers. You will be instrumental in offering guidance and support to help them cope with the emotional ramifications, equipping them with key coping strategies and resources.

Qualifications\n

• A Master's or Doctorate degree in clinical psychology, counseling, or clinical social work from an accredited program (e.g., MSW, MA, MS, MFT, MHC, PhD, PsyD).

• An independent license in the state of Oregon, such as LCSW, LMFT, or LPC.

• Proficiency in electronic communications, especially email, to facilitate seamless coordination with clients and colleagues.

• A proven ability to manage a diverse caseload effectively and communicate effectively with large healthcare providers and support staff.

• A genuine passion for positively impacting the lives of others through behavioral health support.
\n
• Proficient use of electronic medical record systems for monitoring and documentation of patient progress.

Compensation and Benefits\n
