This position is conducted remotely, primarily serving patients enrolled within a school setting in Salinas, California. Key Responsibilities\n

• Manage a designated caseload of students by conducting comprehensive evaluations that assess the social, emotional, financial, and cultural dimensions of each child's life. Formulate and implement tailored treatment plans aimed at alleviating psychosocial barriers to successful educational outcomes.

• Coordinate with multidisciplinary healthcare teams to facilitate care management, addressing complex issues such as protective service needs, life care decisions, and advanced directives for vulnerable students.

• Provide crisis intervention services to students and their families in acute situations, ensuring timely support and guidance through challenging circumstances.

• Lead case reviews and care coordination meetings with community agency representatives to secure adequate resources and referrals for students requiring additional support.

Qualifications\n

Compensation and Benefits\n

The salary for this position ranges from USD 120,000 to 130,000 annually, commensurate with experience and qualifications.
