Key Responsibilities\n

• Conduct thorough psychosocial assessments through interviews and observational methods, along with a review of medical records, to identify emotional, social, and environmental factors that influence patient recovery.

• Deliver compassionate counseling and support to patients and their families, helping them navigate adjustment issues related to their rehabilitation.

• Collaborate with a dedicated rehabilitation team to formulate individualized treatment plans that address both the psychosocial and clinical needs of each patient.

• Assist patients in connecting with valuable community resources including support groups, vocational training programs, and housing assistance to facilitate their reintegration into society.

• Educate patients and their families about available community resources and post-rehabilitation services, empowering them to take an active role in their ongoing care.

• Play an active role in discharge planning processes to orchestrate a smooth transition for patients back to home or other care settings.

Patient Population / Client Focus\n

Your work will primarily focus on children and adolescents within the school system, aiming to provide support that addresses both mental health and developmental challenges. Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
