Work Environment\n

This position is fully remote, allowing for flexibility while maintaining a structured work schedule from 8 AM to 5 PM, Monday through Friday. Key Responsibilities\n

• Conduct comprehensive assessments to determine the physical, emotional, and social needs of clients through interviews and case reviews.

• Develop, implement, and coordinate individualized treatment plans to help clients navigate their challenges and achieve measurable goals.

• Engage in crisis intervention by providing immediate support and guidance during critical situations.

• Maintain accurate documentation of client interactions, treatment plans, and progress reports to track and adapt interventions accordingly.

• Collaborate with multidisciplinary teams and participate in case conferences to develop a holistic approach to client care.

• Facilitate referrals to community resources and support services, empowering clients through access to additional assistance and care.

• Contribute to quality assurance by actively participating in staff meetings and continuous improvement initiatives.
