Key Responsibilities\n

• Conduct thorough psychosocial assessments through patient interviews, observations, and the examination of medical records, aimed at understanding emotional, social, and environmental factors affecting recovery.

• Provide empathetic counseling and support to patients and their families, focusing on adjustment issues related to the recovery process.

• Contribute to the development of individualized treatment plans by collaborating with the multidisciplinary rehabilitation team to consider the unique psychosocial needs of each patient.

• Assist patients in navigating community resources, including support groups, vocational rehabilitation services, and housing assistance, ensuring they have access to the necessary post-rehabilitation support.

• Educate patients and families about community resources, emphasizing the importance of ongoing support and engagement in rehabilitation efforts.

• Engage actively in discharge planning processes to facilitate smooth transitions from our care settings to home or alternative care environments.

Patient Population / Client Focus\n

You will primarily work with patients who have experienced significant life changes due to strokes, spinal cord injuries, orthopedic surgeries, and various neurological disorders. These individuals often face emotional and psychological challenges as they adapt to their new circumstances, making your role in their rehabilitation crucial.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Florida is essential.

Compensation and Benefits\n
