Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through direct patient interviews, clinical observations, and detailed reviews of medical records to identify emotional, social, and environmental factors influencing recovery.

• Provide evidence-based counseling and support to patients and their families, addressing challenges related to adjustment, grief, and emotional well-being during their rehabilitation journeys.

• Collaborate closely with a diverse rehabilitation team to develop and implement individualized treatment plans that address both psychosocial and physical rehabilitation needs.

• Assist patients in accessing essential community resources, such as support groups, vocational rehabilitation, and housing assistance, ensuring robust post-rehabilitation support.

• Educate patients and their families on available community resources and services to facilitate a smooth transition from rehabilitation settings to home or other care environments.

• Actively participate in discharge planning, ensuring continuity of care and supportive reintegration into the community for patients.

Patient Population / Client Focus\n

You will work with a diverse patient population that includes individuals recovering from physical injuries, neurological disorders, post-surgical rehabilitation, and mental health challenges. Qualifications\n

Compensation and Benefits\n
