Key Responsibilities\n

• Engage proactively with participants to effectively avoid reliance on crisis services, establishing supportive relationships that foster trust and rapport.

• Create, maintain, and update crisis alerts to coordinate with county crisis services, ensuring timely intervention and support for participants.

• Coordinate and document transitions in care for hospital or inpatient services, which may include direct communication and collaboration with hospital staff to facilitate smooth discharges and program transfers.

• Monitor and assess client progress, adjusting treatment plans and integrating evidence-based practices to optimize outcomes in mental health performance.

Patient Population / Client Focus\n

In this role, you will serve a diverse population experiencing a range of psychiatric, emotional, and social challenges. Your clients may include individuals coping with anxiety, depression, trauma, relationship issues, and other mental health disorders. You will adapt your therapeutic approach to meet the unique cultural, linguistic, and psychological needs of each client, establishing a safe space that encourages open dialogue and healing.

Qualifications\n

• A Master’s degree in Counseling, Social Work, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• An active and valid state licensure as a Marriage and Family Therapist (LMFT), Licensed Professional Counselor (LPC), or Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

The position offers a competitive salary range of $100,000 to $105,000 annually, depending on your caseload and experience.
