Key Responsibilities\n

• Conduct initial comprehensive psychosocial assessments of individuals, adolescents, families, and groups to determine their specific needs and challenges.

• Provide graduate-level clinical social work or counseling interventions tailored to the unique circumstances of each client.

• Collaborate with the interdisciplinary care team, effectively communicating treatment plans and progress during documentation and meetings, such as patient rounds.

• Demonstrate professionalism and compassion in handling the physical, psychological, social, educational, and safety needs of patients across various age groups.

• Manage cases for assigned patients by consulting and collaborating with community professionals and programs to ensure comprehensive care delivery.

• Ensure timely and accurate documentation in accordance with departmental requirements, upholding ethical standards at all times.

• Utilize Cognitive Behavioral Therapy (CBT) and other evidence-based treatment modalities to assist patients and families in successfully completing their individualized service plans.

Patient Population / Client Focus\n

Your work will primarily revolve around supporting a diverse group of individuals seeking outpatient mental health therapy. You will engage with clients varying in age and backgrounds, addressing a wide range of mental health concerns. This role is pivotal in providing therapeutic support for individuals and families as they navigate challenges related to anxiety, depression, trauma, and more, ensuring they receive compassionate and tailored care throughout their therapeutic journey.

Qualifications\n

• A valid and current license as a LMFT, LPCC, or LCSW as mandated by the State of California.

• A minimum of 2 years of clinical experience in a mental health counseling setting.
