Key Responsibilities\n

• Develop individualized aftercare plans working closely with clients to ensure a smooth transition post-discharge from treatment programs.

• Maintain ongoing communication with clients’ families and external clinicians to promote comprehensive support and continuity of care.

• Conduct thorough assessments and document care processes meticulously, adhering to company policies including completing biopsychosocial assessments, treatment planning, and progress notes using our secure digital health records.

• Facilitate both group and individual therapy sessions, employing evidence-based modalities tailored to meet the unique needs of your clients.

• Engage in regular clinical supervision sessions to enhance your practice and ensure adherence to best practices in mental health therapy.
