Key Responsibilities\n

• Conduct thorough evaluations of clients to identify mental health needs, risk factors, and appropriate interventions, ensuring timely responses to any indications of crisis.

• Provide psychoeducation to clients and families, facilitating process groups alongside one-on-one therapy sessions to foster understanding and support.

• Develop and implement individualized treatment plans based on assessments, collaborating closely with a multidisciplinary team to integrate care approaches.

• Facilitate family, group, and individual therapy sessions utilizing evidence-based practices that promote healing and growth.

• Document all assessments, therapeutic progress notes, and treatment plans promptly and accurately to comply with clinical and legal standards.

• Maintain ongoing communication with care coordinators to evaluate and refine discharge planning efforts aimed at ensuring patient continuity of care.

• Engage in post-discharge follow-ups to track patient progress and offer additional support as necessary.

• Monitor and assess client safety, reporting any changes in conditions and addressing safety concerns proactively.

Qualifications\n
