Key Responsibilities\n

• Conduct thorough risk assessments for patients, including evaluations for suicide, homicide, or other safety considerations, to develop appropriate intervention strategies.

• Complete accurate and timely documentation, including assessments, treatment notes, and discharge planning, while adhering to organizational policies and legal standards.

• Work collaboratively with other healthcare professionals in a multidisciplinary team setting to ensure comprehensive discharge planning and continuity of care.

• Facilitate therapeutic interventions through individual, group, and family psychoeducational sessions, utilizing evidence-based practices to enhance treatment outcomes.

• Constantly assess the clients' safety status and respond to any changes in their condition or potential hazards in their environment.

• Engage in ongoing development of treatment plans, participating actively in treatment planning sessions and regularly reviewing each client’s progress.

• Provide post-discharge follow-up outreach to assess ongoing needs and ensure continuity of care.

Patient Population / Client Focus\n

You will serve a diverse array of clients, including individuals experiencing mental health issues such as depression, anxiety, PTSD, and substance use disorders. Our patient population typically includes both adults and adolescents from varied socioeconomic and cultural backgrounds. Your ability to empathize and connect with clients will be essential in providing them with the support they need in a therapeutic context.

Qualifications\n

• Must possess a valid license as a Licensed Clinical Social Worker (LCSW), Licensed Marriage and Family Therapist (LMFT), or Licensed Professional Clinical Counselor (LPCC).

• A master’s degree in behavioral sciences or a related field from an accredited institution is required.
