Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and their families during admission and as clinically warranted, employing social work theories to identify and alleviate barriers to care.

• Collaborate with multidisciplinary health teams to develop integrated treatment plans while coordinating referrals to specialty services and community resources for holistic support.

• Assist clients and families in understanding and coping with the psychological and environmental challenges associated with illness through supportive counseling and crisis intervention.

• Manage a caseload of patients with varying levels of clinical acuity, providing individualized support tailored to each client's specific situation.

• Engage in proactive case finding to identify patients requiring psychosocial intervention, contributing to team discussions and interdisciplinary rounds to enhance patient care.

• Provide individual and group therapy sessions focused on mental health challenges, grief, and coping strategies while ensuring continuity of care through thorough progress notes and follow-ups.
