Work Environment

Key Responsibilities

• Conduct thorough evaluations and assessments of each patient's physical capabilities, developing personalized treatment plans based on their unique needs and functional limitations.
• Engage in consultations with healthcare providers, ensuring a seamless continuum of care for patients throughout their rehabilitation journey.
• Implement treatment plans efficiently using evidence-based practices while adhering to patient safety and satisfaction standards.
• Regularly document each patient's progress, treatment outcomes, and charges accurately, maintaining compliance with regulatory and professional standards.
• Collaborate with interdisciplinary teams to ensure comprehensive care, involving patients, families, and caregivers in the treatment and discharge planning processes.
• Participate in departmental leadership initiatives such as community outreach, quality improvement projects, and mentorship of support staff.
• Attend staff meetings and professional development programs, continually enhancing your skills and knowledge in therapeutic practices.

Patient Population / Client Focus

In this role, you will work primarily with patients recovering from orthopedic surgeries, sports injuries, and other musculoskeletal conditions. Our diverse client base includes individuals of all ages and backgrounds, ensuring that you will have the opportunity to impact a wide array of lives through tailored rehabilitation strategies.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited program is required.
• A current State License in Physical Therapy, alongside BLS certification, is mandatory.
• Excellent verbal and written communication skills, with the ability to establish rapport with patients and collaborate effectively with team members.
• Proficiency with basic computer applications, including electronic health records systems.

Preferred Experience

• A minimum of one year of experience in an outpatient rehabilitation setting is preferred.
• Experience working with diverse populations and familiarity with culturally responsive care practices strongly desired.
• Previous involvement in community outreach programs or quality improvement projects is a plus.
