Work Environment

This position is based in a home care setting, where the therapist will travel to patients' residences, ensuring personalized, one-on-one care. Key Responsibilities

As a Physical Therapist in our home care unit, your responsibilities will include:

• Evaluating the status of patients recovering from injuries, diseases, or surgeries in their homes.
• Creating individualized treatment plans that incorporate exercises, stretching techniques, and hands-on therapy to alleviate pain and improve mobility.
• Documenting the patient's progress and modifying treatment plans as needed to ensure therapeutic goals are met effectively.
• Training patients, their families, and caregivers on the use of assistive devices and effective physical therapy techniques.
• Conducting ongoing assessments to reevaluate patient progress and adjust care plans in accordance with evolving patient needs.
• Maintaining compliance with state and federal regulations and delivering care that meets our high standards for quality.

Patient Population / Client Focus

Your expertise will primarily serve a broad range of patients, from children to older adults, each facing unique health challenges. This will include those recovering from orthopedic surgeries, managing chronic conditions, or undergoing rehabilitative care post-hospitalization. Qualifications

• A Bachelor's Degree in Physical Therapy or higher (Doctorate degree preferred).
• A valid State License in Physical Therapy.
