Work Environment

This role is primarily onsite, set within a home care environment where you'll engage directly with patients in their residences. This unique setting allows for personalized treatment plans tailored to each patient's specific needs and living conditions. You'll work weekdays, from Monday to Friday, allowing for regular scheduling and ensuring work-life balance. Key Responsibilities

• Comprehensive Evaluations: Conduct thorough evaluations, including history reviews and specific assessments, to develop accurate diagnoses and customized treatment plans.
• Patient and Family Engagement: Clearly explain evaluation findings and involve patients and their families in goal-setting to enhance cooperation and motivation.
• Treatment Implementation: Demonstrate proficiency in implementing treatment protocols based on evaluation findings and ongoing assessments of the patient's progress.
• Collaborative Consultation: Communicate and collaborate with other healthcare professionals as needed, documenting changes in patients' conditions that may require modification of the care plan.
• Documentation and Follow-up: Ensure timely and accurate documentation of all patient care activities, including initial evaluations, daily encounter notes, progress summaries, and discharge evaluations.
• Time Management: Maintain an organized schedule and meet productivity expectations while actively participating in hospital and community marketing activities.
• Goal Achievement: Contribute toward departmental objectives and comply with all relevant governmental and accreditation standards.
• Continuing Education: Proactively pursue professional development opportunities and serve as a clinical instructor for physical therapy students.
• Customer Focus: Promote an excellent patient experience by demonstrating flexibility and adaptability to meet varying patient needs.
• Access Initiatives: Engage in initiatives aimed at removing barriers to care, utilizing data to enhance patient access.

Patient Population / Client Focus

Your work will be focused on a diverse array of patients who require physical therapy services in the comfort of their own homes. Many of these patients may be recovering from surgery, managing chronic illnesses, or overcoming injuries. As you develop treatment plans, you will be required to consider the unique aspects of their home environments, thereby necessitating a patient-centered approach that maximizes their chances for successful recovery.

Qualifications

• A Doctorate degree in Physical Therapy (DPT) is essential for this role.
• A valid DPT license in the state of Florida is required.
• Current certification in CPR from the American Heart Association's Basic Life Support for Healthcare Providers.
• Strong communication and interpersonal skills to foster relationships with patients and their families.
• A commitment to continuous professional education and mastery of physical therapy practices.

Preferred Experience

• While 2 years of clinical experience is preferred, new graduates are welcome to apply, especially those who have completed supervised clinical rotations.
• Experience in home-based care or outpatient settings will be an asset but is not mandatory.
