Work Environment

This position is based onsite, focusing on delivering care directly in the patient's home. Key Responsibilities

• Direct and oversee physical therapy treatment plans that aim to prevent disabilities, alleviate pain, and restore functional movement.
• Conduct comprehensive evaluations of patients using diagnostic and prognostic tests to assess muscle, nerve, joint, and functional capabilities, tailoring treatment plans to individual needs.
• Supervise physical therapy assistants to ensure that therapeutic interventions align with the prescribed care plan.
• Assess the home environment to recommend modifications that can enhance rehabilitation outcomes and promote patient independence.
• Provide care that includes manual therapy, exercise instruction, and the administration of physical modalities as per physician prescriptions.
• Evaluate, fit, and make recommendations regarding prosthetic and orthotic devices to optimize patient mobility.
• Keep meticulous clinical records, including initial evaluations, care plans, and discharge summaries, to ensure continuity of care and compliance with regulatory requirements.
• Engage in interdisciplinary team meetings to plan and evaluate comprehensive treatment strategies for patients.
• Consult and educate patients and families about physical therapy programs and device usage to promote understanding and adherence to treatment.
• Ensure timely submission of documentation and collaborate with other healthcare professionals for an integrated approach to patient care.

Patient Population / Client Focus

The primary focus of this role is to serve a diverse patient population, particularly individuals recovering from orthopedic surgeries, injuries, or those with chronic musculoskeletal conditions. The therapist will address the unique needs of each client, emphasizing personalized strategies that empower them to regain independence in their daily activities.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited program.
• Current state licensure as a Doctor of Physical Therapy (DPT).
• A minimum of two years of hands-on experience in a clinical setting, preferably in home health or orthopedic rehabilitation.
• Current CPR certification.

Preferred Experience

• Experience in developing patient-centered care plans that reflect the unique preferences and goals of individuals.
• Previous experience in home health care settings, enhancing the ability to conduct evaluations and provide interventions in patients' homes.
• Familiarity with electronic health record (EHR) systems to maintain accurate clinical documentation.

Compensation and Benefits
