Work Environment

In this role, you will be working onsite, primarily in home care settings across The Villages area. This unique environment allows you to develop close relationships with your clients in the comfort of their homes, enhancing the therapeutic relationship. Key Responsibilities

• Conduct thorough examinations and evaluations of patients with orthopedic conditions, functional limitations, and disabilities to develop comprehensive treatment plans tailored to their unique needs.
• Provide expert consultations to other healthcare providers regarding each patient's continuum of care, ensuring cohesive treatment strategies from diagnosis through recovery.
• Engage directly with patients, conducting educational sessions to empower them and their caregivers in understanding their conditions and the necessary therapeutic interventions.
• Implement treatment protocols and continuously assess patient progress, adjusting care plans as necessary and adhering to all applicable laws and regulations governing documentation and patient care.
• Communicate effectively with the interdisciplinary treatment team, maintaining accurate and timely records and ensuring collaboration with referral sources and physicians.

Patient Population / Client Focus

Your primary clients will be individuals dealing with a range of orthopedic issues, including post-operative recovery, chronic pain management, and rehabilitation after injuries. You will work with a diverse patient demographic, including elderly patients and those from various cultural backgrounds, requiring sensitivity and adaptability in your service delivery. You will ensure that your interactions adhere to our rigorous customer service standards, fostering a respectful and supportive environment for all.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited program.
• Current state licensure as a Doctor of Physical Therapy (DPT) by the Physical Therapy Examining Committee.
• Two (2) years of relevant experience in physical therapy, preferably in a home care or outpatient orthopedic setting.
• Current CPR certification to ensure the safety and well-being of all patients.

Preferred Experience

Experience in home health care and familiarity with electronic health records (EHR) systems are preferred, as these skills will enhance your effectiveness in managing patient documentation and facilitating communication with healthcare partners. Experience with interdisciplinary team collaboration will be invaluable as you integrate care plans effectively.
