Key Responsibilities

• Conduct comprehensive evaluations of patients by reviewing their medical history and performing specific assessments to create personalized treatment plans.
• Communicate evaluation results and proposed treatment plans to patients and their families, actively involving them in setting therapeutic goals.
• Demonstrate proficiency in implementing treatment protocols tailored to individual patient needs, and adapt therapies based on ongoing assessment and progress.
• Collaborate effectively with other healthcare professionals for cross-disciplinary care, ensuring documentation of any changes in patient conditions.
• Oversee the discharge planning process, ensuring that all follow-up care is clearly communicated and documented.
• Manage time effectively and meet productivity expectations while maintaining a clear and accurate patient schedule.
• Contribute to departmental goals, ensure compliance with regulations, and promote a culture of accountability and excellence.
• Actively pursue continuing education to stay current with best practices in physical therapy.
• Support patient access initiatives and assist in community-based marketing efforts to promote our services.

Patient Population / Client Focus

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.

• A minimum of 2 years of experience in a physical therapy setting is preferred, although candidates with a strong academic background may be considered.
