Key Responsibilities

• Conduct comprehensive patient evaluations, which includes reviewing medical histories, performing specific assessments, and determining appropriate treatment plans.
• Communicate evaluation findings and treatment strategies to patients and their families. Actively involve them in the goal-setting process to foster collaboration.
• Implement personalized treatment protocols based on evaluation results, incorporating evidence-based practices and adjusting to each patient's unique health circumstances.
• Work closely with other healthcare professionals, consulting and providing recommendations as needed to modify care plans based on patient progress and changes in condition.
• Complete discharge planning and ensure effective follow-up care, maintaining accurate documentation of patient encounters, progress notes, and discharge summaries that reflect the patient's journey through treatment.
• Demonstrate exceptional time management and organizational skills to meet productivity expectations while maintaining an accurate schedule of appointments.
• Participate actively in community outreach and marketing initiatives to promote the benefits of physical therapy and enhance the organization's presence within the local community.
• Contribute to the realization of departmental objectives by complying with accreditation regulations and governmental standards.
