Registered Nurse (RN) - Home Health - $75K-100K per year

Job Overview Registered Nurse (RN) Home Health position at Graham Healthcare Group, located in Folcroft, Pennsylvania, serving Havertown and surrounding areas. The role is full‑time (40 hours per week) with 8‑hour shifts and ongoing employment type as staff. Responsibilities Lead and collaborate as a case manager to provide management and delivery of patient care plans. Deliver patient care visits and non‑visit activities while meeting charting deadlines. Self‑manage time and schedule to ensure consistent patient coverage. Utilize tablets, mobile phones, and EMR software for documentation and communication. Qualifications Active RN license in the state of work. Graduated from an approved school of practical nursing. One year RN experience with clinical competence to deliver quality patient care. Valid driver’s license and ability to drive ~20% of the day to patient locations. Demonstrated experience with tablets, mobile phones and EMR software.
