Key Responsibilities

• Conduct comprehensive evaluations of patients, reviewing medical histories, performing diagnostic assessments, and making informed recommendations for treatment plans.
• Clearly communicate assessment results and treatment strategies not only to patients but also to their families, ensuring their involvement in the goal-setting process.
• Implement treatment protocols tailored to each patient, adjusting as necessary based on their progress and evolving healthcare guidelines.
• Regularly consult with other healthcare professionals regarding patient care, documenting any changes in patient conditions that may necessitate alterations in treatment plans or referrals for specialized consultation.
• Manage discharge planning effectively, ensuring continuity of care through timely follow-ups and thorough documentation of patient interactions, including comprehensive initial evaluations and progress notes.
• Exhibit strong time management skills, meeting productivity benchmarks while contributing to departmental goals and engaging in community outreach efforts for the clinic.
• Engage actively in professional development and continuing education initiatives, serving as a mentor for students in training and enhancing the team's overall clinical capabilities.

Patient Population / Client Focus

As a Full-Time Outpatient Physical Therapist at Swift Health, you will work with a diverse patient population ranging from post-operative orthopedic patients to individuals dealing with chronic pain and neurological conditions. Qualifications

• A Bachelor's degree in Physical Therapy is required, with a preference for candidates possessing advanced degrees.

• Current certification in Basic Life Support (BLS) from the American Heart Association or equivalent.

Compensation and Benefits
