Key Responsibilities\n

• Engage in ongoing communication with physicians and other healthcare team members to report on patient progress, needs, concerns, and potential issues, ensuring coordinated care.

• Maintain comprehensive and accurate clinical documentation, including assessment outcomes and progress notes, in alignment with department policies and regulatory standards.

• Collaborate with interdisciplinary teams to create tailored treatment and discharge plans rooted in psychosocial assessments, facilitating smooth transitions in patient care.

• Deliver counseling and crisis intervention services to patients dealing with emotional and behavioral challenges, employing evidence-based practices to enhance their coping mechanisms.

• Consult with families and support systems to create a holistic treatment approach, ensuring that discharge planning is thorough and meets patient needs.

Patient Population / Client Focus\n

In this role, you'll work primarily with patients in hospice care, focusing on the unique emotional and psychological needs of individuals navigating end-of-life challenges. Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• Active and unrestricted LCSW license in the state of Pennsylvania.

• Minimum of 3 years of relevant experience in an outpatient setting, preferably within hospice or palliative care.

• Proficient in using electronic medical records (EMR) systems, as well as MS Office applications such as Word, Excel, and Outlook.

• A self-directed, highly organized professional with strong interpersonal skills and a commitment to patient-centered care.

• Previous experience working with hospice populations or in similar healthcare environments.

Compensation and Benefits\n
