Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to determine the needs of clients and their families.

• Play an active role in developing and implementing Individualized Family Service Plans (IFSP) to meet the unique challenges faced by families in hospice care.

• Monitor and evaluate outcomes as a key team member, ensuring the quality and effectiveness of care provided.
\n
• Offer guidance and support to families, equipping them with necessary skills and resources to advocate for their child’s development.
\n
• Employ a coaching model and Natural Learning Environment Practices to promote awareness and understanding regarding child development.
\n
• Regularly comply with all federal and state regulations related to clinical social work and behavior health services.
\n
• Develop strong rapport with families and collaborate effectively with other healthcare professionals to foster a supportive care environment.

• Attend training and staff meetings to stay abreast of best practices and continuously improve service delivery.

Patient Population / Client Focus\n

This position focuses on working with children in hospice care, including those with disabilities or developmental delays. Your role will significantly impact these children by supporting their social, emotional, cognitive, and physical development through a compassionate and caregiver-centric approach. By providing tailored interventions, you will use evidence-based practices that facilitate family involvement in the therapeutic process, ensuring a holistic approach to care.

Qualifications\n

• Master's Degree or Doctorate in Social Work from an accredited institution.

• Must hold a valid LCSW license issued by the Department of Public Health.

• Minimum of one year of experience working with children in early intervention settings or related fields.
