Key Responsibilities\n

• Conduct comprehensive biopsychosocial assessments tailored to the mental health, substance abuse, and medical needs of clients, ensuring culturally competent care.

• Deliver behavioral health services, facilitating both individual and group counseling sessions, conducting intakes, and creating treatment plans based on thorough evaluations.

• Perform crisis intervention strategies effectively, providing immediate support and resources to clients experiencing acute mental health issues.

• Create accurate electronic documentation of mental health evaluations, progress notes, and treatment plans, maintaining organized records compliant with healthcare regulations.

• Act as a liaison, connecting clients with local resources, community agencies, and medication assistance programs to enhance their support networks.

• Collaborate with Quality Improvement teams to refine and implement data collection processes that inform evidence-based practice and enhance service delivery.

Patient Population / Client Focus\n

As an LCSW at Thrive Mental Health Services, you will primarily serve clients within a hospice context, focusing on individuals and families navigating the emotional complexities associated with terminal illnesses. Your role will encompass supporting clients through grief, loss, and existential issues while providing them with effective coping strategies and therapeutic interventions.

Qualifications\n

• Possession of a valid and relevant state license as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
