Key Responsibilities\n

• Conduct ongoing assessments to identify and respond to the individual needs of patients, ensuring tailored therapeutic interventions.

• Engage in clear communication with supervisors, direct care staff, and family members to track patient progress and provide comprehensive support.

• Develop and share a tentative schedule with supervisors outlining appointments, home visits, and observations as part of patient care responsibilities.
\n
• Coordinate home visits that encompass in-home observations, chart reviews, staff interviews, and data analysis for a holistic understanding of patient needs.
\n
• Ensure effective implementation of individualized program plans and document ongoing developments in a systematic manner.
\n
• Participate actively in Interdisciplinary Team Meetings to discuss patient status, share insights, and refine treatment approaches.
\n
• Maintain accurate and up-to-date records and progress notes on a monthly and quarterly basis to ensure compliance and facilitate continuity of care.
\n
• Conduct training sessions to certify staff and team members in program plan implementation and documentation standards.
\n
• Perform follow-up interviews, home visits, and observations to ensure continuous support and adaptation of care as needed.

• Actively liaise with the public and mental health community to support development goals and enhance service outreach.

Patient Population / Client Focus\n

In this role, you will work with a broad spectrum of clients, including both adults and youth, addressing various mental health issues ranging from anxiety and depression to substance abuse and trauma. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is essential for this position.
