Key Responsibilities\n

In your role as a Licensed Professional Counselor, you will engage in a variety of responsibilities that directly impact your clients' journey towards mental wellness. You will:

• Conduct thorough assessments and ongoing evaluations to tailor treatment strategies to meet each patient's needs.

• Communicate effectively with supervisors, family members, and other stakeholders to monitor clients' progress and adjust plans as necessary.

• Organize and manage your schedule of appointments, home visits, and follow-up sessions, ensuring all activities align with program objectives.

• Coordinate in-home visitations for comprehensive evaluations, including chart reviews and staff interviews, while analyzing data for optimal client support.

• Ensure that all aspects of the implemented treatment plan are executed with fidelity, monitoring progress continuously.

• Participate actively in interdisciplinary team meetings to share insights and collaborate on enhancing client care.

• Document monthly and quarterly progress notes in compliance with organizational standards.

• Facilitate training sessions for staff to ensure competency in implementing treatment protocols and maintaining accurate documentation.

• Provide continuous follow-up support through interviews and home visits to ensure sustained client engagement and address emerging needs.

• Act as a liaison within the mental health community, advocating for clients and resources to meet developmental goals.

Qualifications\n

Compensation and Benefits\n
