Key Responsibilities\n

In this role, you will be responsible for the following:

• Conduct thorough assessments and develop individualized treatment plans that address the specific needs of clients.

• Maintain regular communication with clients, their families, and healthcare professionals to monitor progress and adjust treatment plans as necessary.

• Schedule and facilitate appointments, including virtual check-ins, home visits, and follow-up sessions to ensure ongoing support.

• Ensure that all client documentation, notes, and reports are completed accurately and in a timely manner in accordance with legal and ethical standards.

• Actively participate in Interdisciplinary Team Meetings to discuss client cases and share insights for coordinated care.

• Organize training and support sessions for fellow staff to ensure the effective implementation of treatment programs and documentation practices.

• Serve as a liaison with local mental health organizations to foster relationships that support community development goals.

• Utilize electronic medical records (EMR) systems to document client interactions and track treatment progress efficiently.

Patient Population / Client Focus\n

The LPC will work with a diverse client population, including adults and youth, each facing varying degrees of mental health challenges. Qualifications\n

To be considered for this position, candidates must meet the following qualifications:

• Hold a Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

Compensation and Benefits\n
