Work Environment\n

This position is performed remotely, utilizing telehealth technologies to connect with patients and healthcare team members. You will enjoy a supportive work culture that promotes collaboration and communication among team members. While the standard schedule is Monday to Friday from 8:00 AM to 5:00 PM, there may be occasional evening shifts to accommodate the unique needs of our patients, ensuring they receive timely support and care.

Key Responsibilities\n

• Conduct thorough mental health and psychosocial assessments, taking into account the impact of medical conditions on patients' mental well-being.

• Develop, implement, and regularly update integrated treatment plans that consider both the psychiatric and medical needs of each patient.

• Provide individual and group therapy sessions using evidence-based therapeutic approaches, modulating techniques to fit each patient's unique circumstances.

• Collaborate closely with medical providers to ensure coordinated care and optimal treatment outcomes, participating in regular case conferences and team meetings.

• Offer crisis intervention services and support to patients during mental health emergencies, ensuring they have immediate access to care when needed.

• Educate patients about the intricate connections between their mental health and physical illness, enabling them to take active roles in the management of their conditions.

• Maintain precise and confidential documentation in adherence to all legal, ethical, and clinic standards for patient records and information.

Patient Population / Client Focus\n

Your primary focus will be on adults struggling with a variety of co-occurring medical and mental health conditions. This includes patients with chronic pain syndromes, those managing autoimmune disorders, and individuals coping with substance use disorders. Your empathetic approach and specialized knowledge will empower these patients to achieve improved mental health and overall well-being.

Qualifications\n

• Master's or doctoral degree in Social Work, Marriage and Family Therapy, Mental Health Counseling, or Psychology from an accredited institution.

• Current licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Marriage and Family Therapist (LMFT) in the state of Colorado.

• Demonstrated proficiency in addressing mental health needs in individuals with co-occurring medical conditions.

• Prior experience in integrated care settings, collaborating effectively with medical professionals is highly preferred.

Compensation and Benefits\n

The salary for this role ranges from USD 75,000 to USD 104,000 annually, commensurate with experience and qualifications.
