Work Environment

This role is set in an onsite home care facility, emphasizing the importance of delivering rehabilitation services in a comfortable and personalized setting. As part of a multidisciplinary team, you will work closely with other healthcare professionals to ensure that our patients receive seamless and integrated care. Your work hours will typically total 40 hours per week, with the possibility of variable weekends to accommodate patient needs.

Key Responsibilities

• Conduct thorough evaluations of patients to identify functional limitations and develop customized treatment plans focused on their unique needs.
• Implement a variety of therapeutic interventions, including exercise regimens, manual therapies, and modalities to improve movement and relieve pain.
• Collaborate with healthcare providers to discuss patients' continuum of care, ensuring optimal outcomes and satisfaction.
• Engage in patient education, informing patients and their families about the treatment process, expected outcomes, and coping strategies.
• Maintain accurate documentation of treatment plans, progress notes, and discharge summaries in compliance with all applicable laws, rules, and professional standards.
• Supervise and mentor assigned support staff, ensuring effective communication and treatment continuity.
• Participate in community outreach initiatives, quality improvement projects, and marketing efforts to enhance service visibility.
• Stay current with industry trends through continuing education and in-service training, sharing knowledge with peers.

Patient Population / Client Focus

You will work primarily with diverse populations, including individuals recovering from orthopedic surgeries, those with chronic disabilities, and older adults needing rehabilitation for mobility issues. Sensitivity to cultural and age-related factors is essential, as is an understanding of how different backgrounds impact healthcare accessibility and patient expectations.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution.
• Active State License in Physical Therapy and Basic Life Support (BLS) certification.
• Proficient verbal and written communication skills with experience in utilizing electronic medical record (EMR) systems.
• Strong organizational and time management capabilities to efficiently manage a varied caseload.

Preferred Experience

• Experience in a home care or outpatient orthopedic setting is advantageous.

Compensation and Benefits
