Peer Navigator - Street Medicine (S Riverside, CA)

Overview Healthcare in Action provides street medicine services, delivering medical care directly to people experiencing homelessness in parking lots, encampments, and parks through a medically equipped van and partnerships with Medicare, Medicaid, hospitals, and health systems. Responsibilities Build rapport with members and coach them to develop strategies for improving health outcomes. Connect members to housing, medical, and social services and advocate for medication and supplies as needed. Coordinate and accompany members to medical and social services appointments outside the street medicine unit. Introduce self‑management tools to increase self‑efficacy. Complete all required documentation promptly, including electronic health record entries, care plan updates, HMIS/HMIS updates, and monthly audit checklists. Occasionally drive the medically equipped van and schedule encounters based on member acuity. Work under the supervision of a social worker to ensure proper escalation and care coordination. Qualifications GED/High School diploma required with 3+ years of relevant experience. Associate’s degree preferred with 2+ years of relevant experience. Bachelor’s degree preferred with 1–2 years of relevant experience. Direct work experience, lived experience, training, or volunteer work with patients experiencing homelessness, mental health or substance use conditions. Relevant experience in community or outreach settings with disadvantaged groups. Strong ability to work outdoors, independently, and as part of a team. Excellent organizational and multitasking skills. Excellent oral communication and interpersonal skills. Reliable transportation, valid driver’s license, automobile insurance, and clean driving record. Computer proficiency and experience with online timesheets, call center software, and HMIS/Coordinated Entry System. Willingness to travel 50%+ of work time.
