Peer Navigator, Street Medicine (Redwood City, CA)

At Healthcare in Action, we serve patients experiencing homelessness through an innovative street medicine approach. We contract with Medicare and Medicaid health plans, hospitals, and health systems to deliver medical care directly to people experiencing homelessness in parking lots, encampments, and parks. We seek Rebels who are curious about AI and its power to transform how we operate and serve our members. This is a patient‑facing role that supports individuals in Redwood City, CA, primarily onsite 4 days per week at the San Mateo Navigation Center and 1 day remote. Responsibilities Build rapport with members by communicating personal experiences to increase the likelihood of positive behavior changes. Coach members to create and implement strategies that minimize risks associated with identified health conditions and behaviors. Connect members to appropriate programs to address barriers to care and enhance compliance. Link members to local, county, and state services and resources. Follow up with members and serve as an advocate, delivering medications and supplies as needed. Conduct regular outreach to onsite patients at the Navigation Center and off‑site patients as needed, providing support for medical needs. Accompany patients to medical, dental, and other appointments. Collaborate actively with staff of the Navigation Center and the Healthcare in Action team for care management of patients. Introduce tools to promote self‑management and self‑efficacy. Complete required documentation in the electronic health record system, update the Plan of Care after each street medicine encounter, document encounter notes, submit Monthly Audit Checklist assessments, individual care plans, and update member profiles in the Coordinated Entry System/HMIS. Schedule encounters based on member acuity using the Healthcare in Action electronic health record system. Work under the supervision of a Social Worker to ensure proper escalation of cases and coordination of care. Support justice‑involved clients by collaborating with Correctional Health staff and assisting individuals in custody in county jail with their transition back into the community. Qualifications and Experience G.E.D. or High School diploma required, plus 3+ years of work experience as defined below. Associate’s Degree plus 2 years of work experience. Bachelor’s Degree preferred plus 1+ year of work experience. Work experience to include direct work experience, lived experience, training, or volunteer work with patients experiencing homelessness, as well as those with active mental health and substance use conditions. Other relevant experience in similar community settings will be considered. A comparable combination of education, experience, and/or training will be considered equivalent to the education listed above. Ability to work in a dynamic, outdoor environment; independently and as part of a team. Ability to organize effectively and prioritize multiple competing tasks. Excellent oral communication and strong interpersonal skills; ability to articulate personal experiences. Proficiency in operating a computer, including time‑keeping and call center software; preferred experience with HMIS/Coordinated Entry System. Must have a valid driver’s license, clean driving record, automobile insurance, reliable transportation. Tuberculosis screening: proof of a negative screening within the past year is required upon hire. Other duties as assigned. Benefits Base salary range: $23.51–$35.65 per hour.
