Key Responsibilities\n

• Conduct thorough mental health assessments of patients and their families, considering unique circumstances and presenting concerns to identify emotional and psychological needs.

• Develop and implement individualized treatment plans focusing on mental health issues, emotional support, and effective coping strategies, ensuring alignment with the overall patient care plan.

• Utilize evidence-based therapeutic modalities, such as cognitive-behavioral therapy (CBT) and solution-focused therapy, tailored to the unique needs of hospice patients.

• Provide individual and family therapy sessions via telehealth, guiding families through the emotional challenges associated with end-of-life care.

• Collaborate with a multidisciplinary healthcare team, including physicians, nurses, and chaplains, to coordinate comprehensive care aligned with patients' goals and values.

• Offer crisis intervention support during emotionally challenging situations, employing de-escalation techniques and making appropriate referrals to higher levels of care if necessary.

• Deliver psychoeducation to patients and family members about mental health, coping mechanisms, and available resources within the community.

• Maintain accurate documentation of patient assessments, treatment plans, progress notes, and other essential records, following strict legal and ethical standards of practice.

Qualifications\n

Compensation and Benefits\n

The expected salary for this role ranges from $80,000 to $100,000 annually, depending on experience and qualifications.
