Work Environment

This full-time position operates primarily in a home care setting, allowing you to provide personalized therapy to patients in their own environments. You will have the opportunity to interact closely with patients, ensuring that treatments are tailored not only to their physical needs but also to their home dynamics. Your schedule will be from Monday to Friday, 8 AM to 5 PM, enabling a work-life balance while contributing to meaningful patient outcomes.

Key Responsibilities

• Conduct thorough evaluations of patients to assess their physical therapy needs, utilizing a variety of assessment tools and methods that cater to specific orthopedic conditions.
• Develop and implement customized treatment plans based on each patient's evaluation findings, focusing on achieving measurable outcomes and enhancing functional independence.
• Maintain accurate documentation of patient progress, ensuring compliance with all state and federal regulations while reflecting the patient's course of care from initial assessment to discharge planning.
• Collaborate with a multidisciplinary healthcare team, including physicians and nursing staff, to ensure coordinated care transitions and to address any changes in a patient's condition proactively.
• Provide discharge planning and post-therapy recommendations, ensuring patients are equipped with the strategies and tools needed for continued recovery at home.
• Engage in continuous education and attend facility meetings to remain informed about best practices, regulatory changes, and advancements in physical therapy methodologies.
• Foster open communication with patients, families, and co-workers, demonstrating empathy and professionalism while addressing inquiries and concerns.

Patient Population / Client Focus

You will focus on a diverse range of orthopedic patients, including post-surgical recovery, sports-related injuries, and chronic pain management. Your expertise will be essential in developing strategies that empower patients to take an active role in their recovery journey, thus directly impacting their rehabilitation outcomes.

Qualifications

• A Doctorate (DPT) in Physical Therapy from an accredited institution.
• A valid physical therapy license in the state of Florida.
• Current certification in Basic Life Support (BLS) for Healthcare Providers by the American Heart Association.
• A commitment to continuous professional development and compliance with all regulatory requirements.

Preferred Experience

The ideal candidate will have a minimum of two years of experience working in an outpatient orthopedic setting, demonstrating proficiency in therapeutic techniques and treatment modalities that align with our practice's standards and goals.
