Licensed Physical Therapist - Geriatric Rehabilitation Specialist

Position Overview

We are looking for a dedicated and licensed Physical Therapist (PT) to join our compassionate team at a Skilled Nursing Facility (SNF) located in Chantilly, Virginia. In this full-time role, you will focus on providing tailored rehabilitation services that cater to the unique needs of our residents, helping them recover from injuries, manage chronic conditions, and significantly enhance their mobility and independence.

Work Environment

As a Physical Therapist at our Skilled Nursing Facility, you will work onsite in a collaborative environment. The facility features state-of-the-art rehabilitation equipment and resources, and you will be supported by a team of professional peers, including nursing staff and occupational therapists. The work hours for this position are Monday through Friday, from 9 AM to 5 PM, providing a structured schedule that promotes a healthy work-life balance.

Key Responsibilities

In this role, you will:

• Conduct comprehensive physical therapy assessments to evaluate residents' mobility, strength, and functional capabilities, identifying specific rehabilitation needs.
• Develop and implement individualized treatment plans aimed at alleviating pain, improving mobility, and implementing strategies for fall prevention.
• Deliver hands-on therapeutic interventions, including manual therapy techniques, targeted therapeutic exercises, and specialized balance training.
• Utilize and recommend assistive devices, mobility aids, and adaptive equipment, ensuring residents can maintain their independence in day-to-day activities.
• Work closely with nursing staff, occupational therapists, and caregivers to ensure a seamless and unified approach to resident care.
• Educate residents and their families on home exercise programs, mobility techniques, and safety measures to promote a safe living environment.
• Maintain accurate and timely documentation adhering to all SNF regulations and standards, contributing to a high-quality continuum of care.

Patient Population / Client Focus

This position focuses primarily on the geriatric population, with residents often recovering from surgeries, managing chronic illnesses, or experiencing age-related mobility issues. You will play a critical role in enhancing the quality of life for our seniors by promoting safe mobility, preventing falls, and supporting their rehabilitation goals.

Qualifications

The ideal candidate will possess:

• A valid and active Physical Therapy license in the state of Virginia.
• A Doctor of Physical Therapy (DPT) degree or a Bachelor's/Master's degree in Physical Therapy from an accredited program.

Preferred Experience

Preferred qualifications include:

• A minimum of two years of experience working in a Skilled Nursing Facility or a geriatric rehabilitation setting, showcasing a proven track record in effectively managing patient care.
• Familiarity with long-term care regulations and best practices in rehabilitative care specific to the older adult population.
