Work Environment

This position is based onsite at our Centreville facility, where you will engage directly with patients in their homes. Our home care program provides the flexibility for therapists to address individual patient needs effectively and in a familiar setting. Key Responsibilities

• Conduct comprehensive evaluations of patients to assess functional limitations and disabilities, creating personalized treatment plans to meet their unique needs.
• Deliver therapeutic interventions including but not limited to exercises, manual therapy, and patient education, while documenting each session accurately to track progress and outcomes.
• Collaborate with physicians and healthcare providers to discuss patient care plans and provide regular updates on patient progress, ensuring continuity of care.
• Engage with patients and their families throughout the treatment process, offering guidance on home exercise programs and discharge planning.
• Participate in interdisciplinary team meetings to share insights, improve treatment protocols, and enhance patient care practices.
• Enhance community outreach initiatives and contribute to quality improvement projects aimed at advancing patient care standards.
• Supervise and mentor support staff, fostering a collaborative environment conducive to team learning and development.

Patient Population / Client Focus

As an Orthopedic Physical Therapist at Stalwart Rehabilitation Center, you will work primarily with patients recovering from orthopedic surgeries, injury rehabilitation, and chronic pain management. You will tailor your therapeutic strategies to accommodate diverse patient demographics, including varying ages and cultural backgrounds, thus ensuring that all patients receive equitable and compassionate care.

Qualifications

• A Doctorate in Physical Therapy is required.
• A valid state license in Physical Therapy and current BLS certification are mandatory for this position.
• Strong verbal and written communication skills are essential to effectively engage with patients and collaborate with a multidisciplinary team.
• Proficiency in electronic documentation and familiarity with EMR systems to maintain accurate patient records.

Preferred Experience
