Key Responsibilities\n

• Conduct thorough initial assessments of incoming patients, evaluating their strengths, challenges, and specific needs to formulate effective treatment plans.

• Utilize appropriate therapeutic modalities—including cognitive-behavioral therapy, mindfulness, and other evidence-based practices—to engage clients in meaningful ways.

• Identify and implement psychological interventions tailored to assist clients in achieving their personal goals and improving their emotional well-being.
\n
• Provide individual and family counseling sessions via video or phone, ensuring a supportive environment that facilitates open communication.
\n
• Offer vocational and experiential coaching as needed, helping clients develop skills and strategies for coping with their unique circumstances.

• Collaborate with community resources, referral sources, and medical professionals to coordinate comprehensive care for clients.
\n
• Document all assessments, interventions, and client interactions accurately and in compliance with organizational policies and state regulations.
\n
• Provide on-call crisis intervention and support to clients and their families when immediate assistance is required.
\n
• Maintain oversight of your licensure and engage in ongoing professional development.
\n
• Participate in treatment team meetings, sharing insights and feedback regarding patient progress and treatment efficacy.

Your primary focus will be on patients and families seeking hospice care, where emotional support and psychosocial interventions are paramount. You will work with individuals facing terminal illnesses and their loved ones, providing grief support and helping them navigate the complexities of end-of-life care. Understanding the unique challenges faced by this population is critical in delivering compassionate and effective services.

Qualifications\n

• A Master’s Degree in Social Work from an accredited graduate program.

• Minimum of 2 years of experience in a behavioral health setting, with a strong foundation in mental health practices.

• Current Florida BBS Licensed Clinical Social Worker (LCSW) certification.
\n
• Basic Life Support (BLS) certification from the American Heart Association.
\n
• Excellent critical thinking and organizational skills, with the ability to assess situations promptly and effectively.

• Proficient knowledge and application of assessment, interviewing, and counseling techniques.

Preferred Experience\n

While not mandatory, candidates with prior experience working in hospice settings or with palliative care will be viewed favorably. Familiarity with telehealth practices and electronic health record systems will also enhance your candidacy.

Compensation and Benefits\n

We offer a competitive annual salary ranging from $80,000.00 to $90,000, commensurate with experience, in addition to a robust benefits package that includes:
