Work Environment\n

This role is primarily remote, allowing for flexible scheduling to better accommodate both our employees and clients. Key Responsibilities\n

• Conduct ongoing assessments and comprehensive evaluations to determine and address the unique needs of each patient.

• Maintain clear communication with supervisors, direct care staff, family members, and guardians to facilitate support and monitor individual progress.
\n
• Develop and provide a tentative schedule to supervisors for appointments, home visits, and observations, ensuring thorough monitoring of care delivery.
\n
• Coordinate and execute home visits as necessary to meet the demands of in-home therapy, including observation, chart reviews, and data analysis.
\n
• Implement and evaluate the effectiveness of the program plan while ensuring compliance with legal and ethical standards in mental health practice.
\n
• Participate in Interdisciplinary Team Meetings to share insights and strategies that improve patient outcomes.
\n
• Prepare timely month-end and quarterly progress notes to document patient advancements and areas needing attention.
\n
• Manage training sessions aimed at certifying staff and other team members in rendering effective therapy and completing necessary documentation.
\n
• Conduct follow-up interviews, home visits, and observations to provide continuous support tailored to patients' evolving needs.
\n
• Act as a liaison with the community and mental health stakeholders, aiding in the development of available services.

• Contribute to the creation of procedures and training materials to enhance program effectiveness for individuals receiving care.

Patient Population / Client Focus\n

This role encompasses working with a diverse client population, including both adults and youth. Our LPC will assess and treat a wide array of mental health challenges, focusing on areas such as family dynamics, trauma recovery, coping mechanisms, and substance misuse. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Licensed Professional Counselor (LPC) in the state of Pennsylvania.

• Minimum of 2 years of professional counseling experience, particularly in a clinical setting.

• Specialization or experience in family therapy, couples therapy, substance misuse, or trauma care is highly desirable.
