Key Responsibilities

• Conduct thorough evaluations of patients, including the review of medical histories and physical assessments, to develop individualized treatment plans that meet the specific needs and goals of each patient.
• Effectively communicate evaluation results and treatment strategies to patients and their families, ensuring their active participation in goal-setting and care decisions.
• Implement evidence-based treatment protocols tailored to patients' presentation, adjusting based on ongoing assessments of their condition and response to treatment.
• Collaborate with interdisciplinary healthcare teams, documenting significant changes in patient conditions that necessitate modifications to existing treatment plans.
• Manage discharge planning and ensure continuity of care by following up with patients post-treatment, while also maintaining precise and complete documentation of all patient encounters.
• Demonstrate strong time management skills to meet productivity targets, maintaining an orderly schedule and contributing to outpatient marketing initiatives.
• Participate actively in team meetings to strategize and achieve departmental goals, all while adhering to regulatory standards.

Patient Population / Client Focus

As a Full-Time Physical Therapist at Swift Health, you will work with a diverse outpatient population, including individuals recovering from surgeries, injuries, and chronic conditions. Qualifications
