Key Responsibilities

• Conduct thorough evaluations to assess patients' physical conditions, utilizing performance assessments and a review of pertinent medical histories to formulate individualized treatment plans.
• Communicate evaluation findings and detailed treatment plans to patients and their families, ensuring clarity and understanding, while also involving them in goal-setting.
• Implement evidence-based treatment protocols that are informed by the initial evaluation, adapting as necessary based on patient progress and healthcare innovations.
• Collaborate with interdisciplinary teams to enhance patient care, recognizing and documenting changes in patients' conditions that may necessitate modifications to their treatment plans.
• Oversee discharge planning and follow-up care, ensuring accurate and timely documentation of all patient interactions to create a clear and comprehensive record of their treatment course.
• Manage time effectively to meet productivity standards, maintain an organized schedule, and contribute to departmental objectives in alignment with organizational policies.
• Take an active role in community outreach and marketing initiatives to promote our services and educate the public about the benefits of physical therapy.
• Engage in ongoing professional development and continuing education opportunities in order to enhance personal competencies as well as those of the entire team.

Patient Population / Client Focus

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited program.
• A valid Licensed Physical Therapist (LPT) in the state of New Jersey or eligibility for licensure.
• Completion of the American Heart Association's BLS for Healthcare Providers CPR certification.
• A minimum of 2 years of experience in a clinical setting, particularly in outpatient therapy, is preferred.

Preferred Experience

While a minimum of 2 years of experience is required, candidates with specialized training in areas such as sports rehabilitation, geriatrics, or orthopedics will be given preference. Experience with electronic medical records (EMR) systems and familiarity with evidence-based practice guidelines will be advantageous.
