Key Responsibilities\n

• Conduct comprehensive bio-psychosocial assessments for mental health, substance abuse, and medical needs to develop tailored treatment plans.

• Facilitate individual and group counseling sessions, incorporating evidence-based therapeutic techniques to engage clients in their healing processes.

• Document mental health evaluations, progress notes, and crisis intervention responses efficiently in electronic health record systems.

• Perform suicide risk assessments and escalate concerns to supervisors and medical professionals as warranted, ensuring all patient needs are met rapidly and effectively.

• Coordinate and provide referrals to local resources, such as community agencies and medication assistance programs, to support clients in achieving their treatment goals.

Patient Population / Client Focus\n

In this role, you will work primarily with a diverse client base within the community health clinic setting. Qualifications\n
