Key Responsibilities

• Conduct thorough evaluations of patients in the home care environment, assessing their condition, functional limitations, and specific needs.
• Develop and implement individualized treatment plans using evidence-based practices and a variety of intervention techniques such as therapeutic exercises, hands-on therapy, and mobility training.
• Monitor and document patient progress meticulously, adjusting treatment goals and strategies as necessary to align with patient recovery.
• Educate patients, caregivers, and family members on effective therapeutic techniques, fostering a collaborative approach to rehabilitation.
• Assist in recommending and fitting patients with appropriate assistive devices to maximize their independence and quality of life.
• Engage in ongoing professional development through educational workshops and collaboration with interdisciplinary teams, ensuring the highest standard of care.

Patient Population / Client Focus

Our patient population ranges from pediatric to geriatric individuals, encompassing various diagnoses including post-surgical rehabilitation, neurological disorders, and chronic pain management. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A valid state license to practice as a Physical Therapist.
