As a Critical Care Specialist, you will engage in virtual consultations with clients and interdisciplinary team members, collaborating closely with healthcare professionals to develop comprehensive care plans. Key Responsibilities\n

• Conduct thorough psychosocial assessments by obtaining detailed information about patients' and families' emotional, social, and psychological states.

• Deliver individual, family, and group therapy sessions tailored to the unique needs of each client, employing evidence-based treatments such as cognitive-behavioral therapy and solution-focused brief therapy.

• Collaborate with multidisciplinary teams, including nurses, doctors, and spiritual care providers, to create and implement integrated, patient-focused care plans.

• Assist in crisis intervention and risk assessment, providing safety planning to ensure the well-being of patients during challenging transitions.

• Document clinical interactions and treatment plans thoroughly to maintain compliance with ethical and legal standards.

• Lead engaging group therapy sessions and psychoeducation classes to foster community support and understanding among patients and families.

• Participate in regular case consultations, sharing insights and strategies to improve therapeutic outcomes and patient satisfaction.

Patient Population / Client Focus\n

Your work will focus on a diverse clientele, including individuals with terminal illnesses and their families. You will help patients manage the psychological challenges associated with end-of-life issues, such as grief, anxiety, and feelings of hopelessness. By facilitating open conversations and providing emotional support, you will empower families to make informed decisions, ensuring their needs and wishes are respected during the hospice process.

Qualifications\n

• Master's degree in Social Work from an accredited program.

• Current and active state licensure as a Licensed Clinical Social Worker (LCSW) is required.

• A minimum of two years of experience in a mental health or outpatient clinical setting, preferably working with end-of-life care or hospice populations.
