Work Environment\n

This position is fully remote, offering flexibility while maintaining a structured work schedule. The work hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours as necessitated by client needs. Our team values flexibility, allowing you to customize your schedule to align with client appointments and follow-ups. While this role is largely remote, the Social Worker will engage regularly with a multidisciplinary team, utilizing telehealth technologies and electronic health record (EHR) systems to ensure seamless collaboration and care coordination.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial dynamics, evaluating emotional, mental, and social factors to identify individual needs.

• Collaborate with seniors and their families to create personalized care plans that align with their specific goals, preferences, and situations.

• Offer empathetic counseling and emotional support to seniors as they confront the complexities of aging, life transitions, and other challenges.

• Advocate for clients to ensure their rights are protected and that they have access to essential community resources and services.

• Work within a collaborative interdisciplinary framework, partnering with nurses, caregivers, and other healthcare professionals to enhance overall client care.

• Regularly visit clients (virtually or in-person as needed) to monitor well-being, evaluate living conditions, and address any safety or health concerns.

• Facilitate open communication among family members involved in the care of the clients, helping to resolve conflicts and ensure everyone is on the same page.

• Educate clients and families about community resources, support groups, and programs that can foster social engagement and improve quality of life.

• Maintain accurate and timely documentation of clients' assessments, care plans, progress notes, and all correspondences.

• Stay updated on best practices, policies, and regulations to ensure compliance with ethical standards in social work.

Patient Population / Client Focus\n

The primary focus of this role is to serve seniors and their families within the community. Understanding the distinctive challenges faced by aging individuals, the Social Worker will provide tailored services that address the complexities of mental health, social isolation, and the need for supportive resources as they navigate different life stages. The Social Worker will engage with clients who may be experiencing a variety of life transitions, including health declines, loss of loved ones, and adjustments to living arrangements.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution is mandatory.

• Current and valid licensure as a Licensed Master Social Worker (LMSW) in the state of Florida is required.

• A minimum of two years of experience in a social work role, preferably with a focus on geriatric care or within a community health setting.

• Strong understanding of social work principles, particularly as they relate to aging and the community resources available for seniors.

• Exceptional assessment, communication, and interpersonal skills, coupled with a natural ability to empathize with seniors and their complex needs.

• Familiarity with using EHR systems and digital platforms for remote service delivery.

Preferred Experience\n

Compensation and Benefits\n
