Work Environment

This position is structured in an onsite home care environment in Columbus, where you will visit patients in their homes, assess their needs, and implement care plans tailored to each individual. The typical workweek will consist of 36 to 40 hours, with flexibility in scheduling to meet our patients' needs.

Key Responsibilities

• Conduct thorough patient assessments, both physical and psychological, to understand acute and chronic medical conditions.
• Develop and implement evidence-based treatment plans by collaborating with physicians, nurses, and other allied health professionals.
• Order, conduct, and interpret diagnostic tests, using findings to establish appropriate medication regimens and therapeutic interventions.
• Educate patients and their families on managing their health conditions, ensuring understanding of medication adherence and care protocols.
• Engage in interdisciplinary meetings and home visits, actively participating in team discussions to optimize patient care strategies.
• Utilize electronic health record (EHR) systems to maintain accurate documentation of patient care and treatment progress.
• Participate in quality improvement initiatives, contributing insights and recommendations based on clinical experiences and outcomes.
• Keep abreast of current best practices, healthcare regulations, and technological advancements in the field of nursing.

Patient Population / Client Focus

The Nurse Practitioner will primarily work with a diverse patient population, including elderly individuals, those recovering from surgery or acute health events, and patients with chronic illnesses requiring ongoing management. You will be vital in helping patients transition from hospital to home care, ensuring continuity of care that leads to improved health outcomes and a better quality of life.

Qualifications

• Current certification as a Nurse Practitioner (NP) is essential.
• An active nursing license in the state of Georgia is required.
• A Bachelor’s degree in Nursing is mandatory, with a Master’s degree preferred.
• Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in home care or outpatient settings.
• Excellent communication and interpersonal skills to foster effective relationships with patients, families, and colleagues.

Preferred Experience

Compensation and Benefits

Gotham Enterprises offers a competitive annual salary ranging from USD 120,000 to 220,000, commensurate with experience and qualifications.
